In patients with a pre-existing tracheostomy undergoing sternotomy with cardiopulmonary bypass, what is your usual airway approach prior to incision to minimize contamination/infection risk?
A. Leave the tracheostomy in place (no change)
B. Switch to a cuffed tracheostomy
C. Replace tracheostomy with an endotracheal tube (ETT) in the stoma and replace native tracheostomy at the end of the procedure
D. Remove tracheostomy, intubate from above and have surgical team temporarily close the stoma (purse string suture)
